Proof of Citizenship Information

Proof of Citizenship is required when leaving the United States.  Failure to present citizenship documentation at time of airport check-in, will result in denied boarding.  If you have any questions regarding required citizenship, contact our office.  We will be glad to help!

 

For Natural-Born U.S. Citizens:

     A valid passport OR

     A certified state-issued birth certificate (not hospital issued) AND a valid, non-expired government-issued

          photo ID (both are required).

 

For Naturalized U.S. Citizens (including minors accompanied by both parents):

     A valid passport OR

     A valid, non-expired government-issued photo ID AND on of the following:

          - A certified state-issued birth certificate (not hospital-issued)

          - Consular Report of Birth Abroad of a U.S. citizen.

          - Other official U.S. government document verifying citizenship.

 

For Non-U.S. Citizens:

     A valid passport.

     A visa issued by the consulate of the destination country (if required).

     A certified naturalization certificate AND a valid non-expired government-issued photo ID.

 

For Married or Divorced Women:

     If ANY ONE travel document (valid passport, state-issued birth certificate, photo ID compared to each other and/or travel itinerary/air

        tickets) shows a differing last name, they must also possess a marriage license or divorce decree.

     If ALL travel documents show the same last name, a marriage license or divorce decree is not required.

 

For Minors Traveling Alone:

     A valid passport and a notarized letter of consent submitted by both parents OR

     A certified state-issued birth certificate (not hospital issued) and a notarized letter of consent submitted by both parents.

 

For Minors Accompanied By One Parent:

     A valid passport OR

     A certified state-issued birth certificate (not hospital-issued).

     Accompanying parent must hold a notarized letter of consent from the absent legal parent.  If parents are divorced, accompanying

          parent must possess a copy of a legal document giving him/her sole custody of the minor child.  If one parent is deceased, a

          copy of the death certificate is required.

 

For Minors Accompanied By a Legal Guardian:

     A valid passport OR

     A certified state-issued birth certificate (not hospital-issued).

     Accompanying legal guardian must possess a copy of a legal document giving him/her sole legal guardianship of the minor child.

 

** Note - A minor child is anyone under the age of 18, including infants.
